A Sport’s Physical is mandatory before Middle School/High School students participate in any sports in the upcoming school year.  Students will not be allowed to even practice for a sport without a physical.  Sport’s Physicals are required every two years.  If your student remains athletically active, you can expect to renew their physical during 6th, 8th, 10th and 12th grades.  Enclosed you will find the OSAA approved physical form.  Please be sure to use this form when you visit the doctor.
